In order to provide consent, Parents must sign this consent form electronically and verify that the person giving the consent is the child’s parent. So, parents providing consent must follow this two-step process:

    1. Signed this consent form and send it back to Educated Soccer to keep in the records (automatically sent once completed).  Then, 
    2. Make a payment with a credit card in order to verify the name of the parent providing consent.

If the parental consent is not completed within 7 days, this website will delete the parent’s online contact information from your records and the child will not have any access to this website.
